What Is a Biomarker? Seven Types in Plain English

What Is a Biomarker?
A biomarker is a defined, measurable characteristic used as an indicator of a biological process, a disease process, or a response to an exposure or intervention. It may be molecular, histologic, radiographic, or physiologic. The useful question is not simply “What was measured?” but “What is this measurement meant to indicate in this setting?”
That definition comes from the FDA-NIH Biomarkers, EndpointS, and other Tools resource, usually shortened to BEST. It keeps the term broader than “blood test” and narrower than “anything we can count.” A scan feature, tissue characteristic, gene variant, or physiological measurement can be a biomarker when it has a defined indicator role.
A biomarker is a job description, not a fancy synonym for a laboratory result.
What Are the Seven Biomarker Categories?
The U.S. Food and Drug Administration lists seven BEST categories. The category describes what the marker is being used to do, so the same measured characteristic may have different roles in different contexts.

| Predictive | Which individuals are more likely to have a favorable or unfavorable effect from a medical product or environmental agent. |
| Pharmacodynamic or response | That a biological response, beneficial or harmful, occurred after an exposure to a medical product or environmental agent. |
| Safety | The likelihood, presence, or extent of toxicity after exposure to a medical product or environmental agent. |
These labels can overlap. A measurement repeated during a study may be a monitoring biomarker, while its particular purpose could also make it a safety or response biomarker. Read the stated role rather than assigning a category from the marker's name alone.
What Information Makes a Biomarker Claim Useful?
The FDA says a full biomarker description includes the biomarker's name, its source or matrix, the characteristic being measured, and the analytical method. In plain English: what, from where, and measured how.
Then look for the context of use:
- Population: Who or what was studied?
- Purpose: Was the marker used for diagnosis, monitoring, prediction, safety, or another defined role?
- Timing: Was it measured once, repeatedly, before an exposure, or after one?
- Method: Which assay, scan, or other analytical procedure produced the value?
- Decision boundary: What interpretation was supported, and under which conditions?
This is also where research-model language matters. A biomarker observed in cultured cells answers a question about that experimental system. It does not automatically carry the same interpretation in a living organism or a person.

Is a Biomarker the Same as a Clinical Outcome?
No. The FDA's definition says a biomarker is not a direct assessment of how a person feels, functions, or survives. A clinical outcome assessment and a biomarker therefore answer different questions.
Some response biomarkers are used as surrogate endpoints in trials, but “biomarker” does not mean “proven substitute for a clinical benefit.” BEST defines a surrogate endpoint as a biomarker used in place of a direct measure of how a patient feels, functions, or survives, with an expectation that it predicts benefit or harm based on evidence. The context and validation matter.
What Does FDA Qualification Mean?
In the FDA's drug-development program, qualification means a biomarker has gone through a formal regulatory process for a specific interpretation and application within a stated context of use. The FDA also makes a useful distinction: the biomarker is qualified, not automatically every method that might measure it.
Qualification is therefore not a universal stamp for every population, assay, or purpose. When reading a claim, check the exact context instead of letting the word “qualified” do more work than the evidence.
For a personal test result, the relevant interpretation depends on the test, units, reference information, health context, and reason it was ordered. That question belongs with the clinician responsible for the test, not a general terminology guide.
